Medical Records

Paper copies of medical records may be released upon receipt of written authorization from patients over 18 or their legal guardians. Once your authorized request is received, processing your request may take up to 30 days.

In some cases, copies of medical records requested by a patient or patient representative might incur a charge. However, if the records are needed for continued care, the patient is not charged when records are sent directly to the physician or the facility providing treatment.

If you need medical records for treatment or an appointment with a provider outside of Florida Digestive Health Specialists (FDHS) within 48 to 72 hours, the physician or the facility providing treatment can request them by fax when you arrive at their office. The patient is not charged for faxing records directly to the physician or the treatment facility.
