At Florida Digestive Health Specialists (FDHS), your health and well-being are our top priorities. To help us provide you with the best possible care, we would like to remind you of the following important information:

We accept most health insurance plans; however, it is important for you to verify your coverage before your visit. We recommend you contact your health insurance plan before your appointment to confirm the services covered and ensure that your FDHS provider is in-network with your insurance plan. Also, please bring your insurance card(s) to each visit, as having this information readily available will help us keep your active insurance coverage up to date so we can submit claims for you.

Medicare Patients:

As providers who participate in Medicare, we will file claims for you. As per Medicare’s guidelines, you will be responsible for a 20% co-insurance once you have met your deductible. We will also file with your secondary insurance plan unless you inform us that you do not want us to do so.

Referral and Pre-Authorization:

If your insurance plan requires a referral or pre-authorization before your visit or treatment, please confirm that your insurance provider or primary care physician has fulfilled the necessary requirements before your appointment. If not, you may be responsible for the entire cost of the visit or procedure.

Patient Financial Responsibility:

Patients are responsible for paying co-pays, co-insurance, deductible payments, and self-pay fees at the time of service. For your convenience, we accept most major *credit cards, cash, and personal checks. * Please note that a 3% processing fee will be applied to all credit card transactions. There is no processing fee for debit or prepaid credit cards, Health Savings Accounts (HSA card), or Flexible Spending Accounts (FSA card).
